Repeat prescriptions may be ordered by completing our order form. Please ensure you give 72hrs notice to ensure that your prescription is sent electronically to your nominated pharmacy.
Occasionally it is necessary to undertake medication reviews with patients so please ensure that you include your phone number on your request form.
Download FormPrescriptions will be prepared in multiples of one month acute, three months medical card and six months private.
There will be a fee for all Private prescriptions. Admin will call you for payment prior to your prescription being sent to your nominated pharmacy by secure email.
Please complete our Registration Document - ensuring one form is completed & signed for each adult of 16 years and over. Children Under 16 should be included on one of the parent's registration forms.

Consent for Notes should be completed for each adult over 16 and children under 16 should be noted on a parent's request for notes. Please ensure you detail the name and address of your previous Doctor in Ireland.
