Medical Certificates

Requests can be made via reception or through email for Private and Social Welfare medical certifates. An appointment or telephone consultation will be offered for review of illness.

Social Welfare certificates are actioned electronically through the GP Surgery. Should it be your first claim for an illness, an IB1 form should be completed and posted to the Social Welfare address located on the form.

This form can be collected from the surgery or found online at www.mywelfare.ie.

Requirements for GP

• PPS Number - for your social welfare certificate
• Date certificate due.
• Email address

A charge will be applicable for all private certificates issued to both private and medical card patients - see professional fees.
